OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by or crushing hazards.  Maintenance Shop:  On or about April 22, 2015, employees were exposed to struck by or crushing hazards when operating a Yale gantry crane, model #KELI-15H3051, serial #D7180, that did not have the locking latch on the crane hook.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace or repair the locking latch on the overhead gantry crane hook as required by ANSI B30.16.

29 CFR 1910.179(g)(1)(v): Pendant control boxes were not clearly marked for identification of functions:  a)  Maintenance Shop:  The employer failed to ensure that the up/down operation buttons on the pendant control box of an overhead gantry crane, model #KELI-15H3051, serial #D7180, was marked to identify its function.  Rain CII was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.179(g)(1)(v), which was contained in OSHA inspection number 464438, citation number 01, item number 001 and was affirmed as a final order on 08/07/2012, with respect to a workplace located at 700 Coke Plant Rd., Chalmette, LA 70043.  Pursuant to 29 CFR 1903.19, with (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the buttons on the pendant control box is marked to identify its function.
