29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about August 11, 2015, the employer failed to ensure that employees were provided and utilizing head protection while overhead work was being performed. Employees were observed working on both lower levels of a scaffold and the ground level while work was being performed above them. The hazard was observed at a worksite located at 28699 Chagrin Blvd., Woodmere, OH.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide  On or about August 11, 2015, the employer failed to ensure that each working level of the scaffold in use at 28699 Chagrin Blvd., Woodmere, OH, was fully planked. Employees were working from levels containing one single plank to perform plastering and caulking activities.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  On or about August 11, 2015, the employer failed to ensure that employees tasked with working from levels of a walkthrough frame type scaffold system were provided with a means of access to the levels in which they would be working. Employees were observed using the framing to access the levels.
