29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:   a. Construction Site - On or about August 11, 2015, an employee was exposed to a fall hazard when he climbed from the roof of a four story building and into an aerial lift basket.  The employee was lowered to the fourth floor where he climbed out of the aerial lift basket and onto the fourth floor.

29 CFR 1926.453(b)(2)(v): 29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a. Construction Site - On or about August 11, 2015, an employee was operating a Genie aerial lift and he did not fasten his harness straps and he did not connect his lanyard to the aerial lift basket.    b. A second employee was on the roof of a building that was under construction. The aerial lift operator elevated the basket to the roof and the employee climbed from the roof and into the aerial lift basket and was lowered to the fourth floor. The employee was not wearing a harness and lanyard while in the basket.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. Roof -  An employee was standing at the edge of a flat roof that was 46 feet high without having fall protection to keep him from falling from the roof.
