Construction Site - On or about August 11, 2015, an employee was operating a Genie aerial lift and he did not fasten his harness straps and he did not connect his lanyard to the aerial lift basket.    b. A second employee was on the roof of a building that was under construction. The aerial lift operator elevated the basket to the roof and the employee climbed from the roof and into the aerial lift basket and was lowered to the fourth floor. The employee was not wearing a harness and lanyard while in the basket.
