D & L Construction and Vinyl Fencing; 649 Park Avenue, Meadville, PA - On or about 8/11/15, and at time prior thereto, employees on the ground performing ground work were not wearing hard hats to protect from the overhead hazards.  Overhead hazards included but were not limited to, roofing materials, other employees working overhead, and tools.

D & L Construction and Vinyl Fencing, 649 Park Avenue, Meadville, PA - On or about 8/11/15 and at times prior thereto, employees were not utilizing any type of fall protection when working on a roof.  The eave to ground height was approximately 10 feet.

D & L Construction and Vinyl Fencing, 649 Park Avenue, Meadville, PA - On or about 8/11/15, the company did not have a training program in place to address fall hazards, to train employees on fall hazards and to inform employees on how to mitigate the potential of a fall.  Employees were potentially exposed to a fall hazard of at least 10 feet while performing roofing work.

The ladder was not extended 3 feet above the landing platform of the roof.  The ladder extended approximately 1 foot above the roof.

The company performs construction work activities including but not limited to roof work and interior renovation work.
