29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:   a)  On or about 08/11/2015, carpenters were exposed to a possible 10 foot fall hazard from a set of steps not provided with a handrail and which was used to access a building's second floor.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed:   a)  Rear of House - On or about 08/11/2015, workers were exposed to a 6 foot fall from a 6 foot fiberglass portable step ladder that was folded together and which was used to gain access to the first floor of a building.              b)  Second Floor - On or about 08/11/2015, workers were exposed to a 10 foot fall from the top step of a 10 foot fiberglass portable step ladder that was folded together and which was used to gain access to the third floor deck.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:       a)  Second Floor - On or about 08/07/2015, two carpenters were not provided with fall protection and exposed to a 30 foot fall while walking on a 6 inch wide top plate of a wood framed wall.     b)  Third Floor Deck - On or about 08/11/2015, carpenters were not provided with fall protection and exposed to a 30 foot fall while installing plywood sheathing.     c)  Second Floor - On or about 08/11/2015, carpenters constructing wood framing were exposed to a 20 foot fall from a set of three window openings which had not been provided with fall protection.     d)  Second Floor - On or about 08/11/2015, carpenters constructing wood framing were exposed to a 25 foot fall from through a door opening which had not been provided with fall protection.     e)  Third Floor - On or about 08/13/2015, two carpenters were not provided with fall protection and exposed to a 40 foot fall while walking a 6 inch wide top plate of a wood framed wall.      A/X Carpentry, was also previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 316182872, citation number 1, item number 2 and was affirmed as a final order on 07/07/2012, with respect to a workplace located at 5420 Galena Pl., NW, Washington, DC 20016;     A/X Carpentry, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 951243, citation number 2, item number 1 and was affirmed as a final order on 04/07/2014, with respect to a workplace located at 2927 44 Pl., NW, Washington, DC 20005.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
