29 CFR 1910.23(a)(5): 29 CFR 1910.23(a)(5): For every pit and/or trap door floor opening, while the cover was not in place, was not constantly attended by someone or was not protected on all exposing sides by removable standard railings:    (a) Hummelstown Swim Club - Employees cleaning the main pool filter daily were exposed to the hazard of falling into the pit which houses the filter, and adjacent pit(s). The filters are located in a pit that is 6 feet, 4 inches deep and while cleaning employees stand or work near the edge of the pool to rinse the filters. The pit door was left open, and the pit was not being constantly attended to prevent employees from falling into the pit, on or about August 11, 2015.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.151(c): Where the eyes or body of any person was exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was not provided within the work area for immediate emergency use.  (a) Hummelstown Swim Club - The employer did not provide suitable eyewash or facilities for quick drenching for employees exposed to corrosive liquids.  Employees were required to cleaned up 25% sodium hypochlorite solution, a corrosive, after a spill, on or a about August 11, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Hummelstown Swim Club - The employer did not develop and implement a written hazard communications program for employees who handle chemicals such as but not limited to sodium hypochlorite (chlorine), and diatomaceous earth filter media which contains crystalline silica, on or about August 11, 2015.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Hummelstown Swim Club - The employer did not provide information and training to employees who were exposed to sodium hypochlorite (chlorine) and diatomaceous earth (D.E. filter media) which contains crystalline silica while cleaning the filter media, on or about August 11, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
