Wimsatt Building Material Corp. worksite located at 326 Illinois Ave., Maumee, Ohio: On or about August 10, 2015, the employer did not ensure the fork attachment suspended by the Hiab knuckle boom crane was designed to be used as a anchorage point for attaching personal fall arrest equipment. Employees were working from a pallet suspended by the crane near the peak of the structure removing weatherproofing material.
