29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment was not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: A. Wimsatt Building Material Corp. worksite located at 326 Illinois Ave., Maumee, Ohio: On or about August 10, 2015, the employer did not ensure the fork attachment suspended by the Hiab knuckle boom crane was designed to be used as a anchorage point for attaching personal fall arrest equipment. Employees were working from a pallet suspended by the crane near the peak of the structure removing weatherproofing material.
