29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  (a)  In the shop:  On August 11, 2015 and at times prior to; the employer exposed his employees to struck-by hazards in the employer allowed employees to walk and work around unsecured compressed gas cylinders that were not secured and prevented from falling.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least 30 minutes:  (a)  In the shop:  On August 11, 2015 and at times prior to; the employer exposed employees to hazards associated with an oxygenated fire such as smoke and fume inhalation, burns and potential death in that employees were allowed to work around an oxygen compressed gas cylinder that was stored with an acetylene compressed gas cylinder.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily obvious:  (a)  In the shop:  On August 11, 2015 and at times prior to; the employer exposed employees to crushing hazards from an automotive lift, SN: 272995, in that the lock on the lift was inoperative and the lift was not tagged "out of service".
