29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about July 15, 2015, at the above addressed site, an employee was exposed to an amputation hazard while working on a truck tire cutter that had the operation controls situated closely together allowing the operator to move the controls with one arm.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  On or about July 15, 2015, at the above addressed site, the employer did not report the in-patient hospitalization for an amputation to the tip of an employees finger to OSHA within twenty-four hours from learning of the incident.
