29 CFR 1926.25(a): Employees were exposed to slips, trips and falls from the debris left in work area. All debris shall be kept clear from work areas and passageways in and around buildings and other structures.  On the west side of the building, masonry employees installing brick from a (2) tier tubular welded frame scaffold were exposed to trip and fall hazards where the employer did not ensure that the job site was kept clean from debris such as broken bricks, lumber and other scrap material.

29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access:  On the west side of the building, masonry employees installing brick from a (2) tier tubular welded frame scaffold were not provided with a ladder or other safe means of access exposing the employees to a fall hazard.

29 CFR 1926.451(g)(1)(vii): Each employee on a tubular welded frame scaffold more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.                         On the west side of the building, masonry employees installing brick from a (2) tier tubular welded frame scaffold were not provided with fall protection or guardrails exposing them to an unprotected fall hazard of approximately (12) feet to the concrete ground below.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects:  On the west side of the building, employees working on or near a (2) tier tubular welded frame scaffold did nto have toe-boards installed or any other additional protection exposing the employees to being struck by flying or falling objects such as bricks and tools from upper levels.
