On August 11, 2015, and times prior to, employees were exposed to electrical shock hazards while using a cement mixer that did not have its ground permanent or continuous in the following conditions. a) The ground plug on the main power supply was missing. b) The ground wire from the main power supply was severed.

On August 11, 2015, and times prior to, employees were using a cement mixer with a splice on the electrical cord. The splice did not retain the insulation, outer sheath properties, and usage characteristics of the cord being spliced and exposed employees to electrical shock hazards.
