29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)      Defense Logistics Agency building SDA-204: On or about 08/06/2015, subcontracted employees working on a flat roof approximately 20 feet wide were not provided with fall protection while disconnected from the horizontal lifeline as a result an employee fell 15 feet to the ground.    No abatement certification or documentation required.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  a)    Defense Logistics Agency Building SDA-204: On or about 08/06/2015, the employer did not utilize a qualified person to install the Guardian CB-12 Horizontal Lifeline and restraint system used by employees.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a)    Defense Logistics Agency Building SDA-204: On or about 08/06/2015, subcontracted employees were not trained by a Competent Person in the proper procedures of erecting and inspecting the CB-12 Guardian horizontal lifeline and fall restraint systems.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
