OSH ACT of 1970 Section (5)(a)(1): Executive Order 12196 Section 1-201(a): The head of the agency did not furnish to employees places and conditions of employment that are free from recognized hazards that are causing or are likely to cause death or serious physical harm in that the employees were exposed to struck by hazards. Location: 12 Arlington Ave., Bloomfield, NJ. On or about 8/6/15; a) An employee was pinned between a GOYA delivery trailer truck's rear bumper and a 99 Cent Depot's loading dock. The employee was attempting to open the truck's rear sliding door while the truck was backing up to the loading dock.
