29 CFR 1910.151(a): Medical personnel were not readily available for advice and consultation on matters of plant health.    On or about 11 August 2015, employees working in the cookie production area were exposed to excessive levels of heat when conducting tasks such as, but not limited to batching, lifting, packing, palletizing and operating the mechanical mixer.    Among other methods, some feasible methods to correct this violation include, but are not limited to :  1. Implementing a work/rest regimen in accordance with the American Conference of Governmental Industrial Hygienists (ACGIH) 1992-1993 Permissible Heat Exposure Threshold Limit Values that will account for temperature extremes and the different shift lengths. Employees on 8 hour shifts cannot have the same work rest cycle as those on 10 hour shifts.   2. Acclimatizing employees beginning work in hot environments or those returning from an absence period of three or more days by grading work tasks.  3. Providing a training program for all employees regarding the health effects associated with heat stress, recognizing symptoms of heat-induced illnesses and methods of preventing such illnesses and making sure that the program is effectively enforced.  4.Encouraging employees to drink 5 to 7 ounces of cool water every 15 to 20 minutes, especially during the summer months when it is hotter than usual. Lead employees should be retrained to ensure the provided Gatorade water coolers are refilled whenever they are empty.  5. Medical surveillance of employees, especially those on chronic medications that may alter anatomical metabolic activities as well as water excretion.   6. Providing PPE such as ice vests or neck towels that can maintain the body's core temperature to below 98 degrees Fahrenheit.   7. Designating a location such as the air conditioned break room where employees can take cooling breaks on a rotational basis to meet production needs.   8. If possible, increase air movement by utilizing fans but only if the air temperature is below 98 degrees Fahrenheit.

29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings on all open sides.    On or about 11 August 2015, employees working on the mixer platform were exposed to a fall hazard of ~4.6 feet to the concrete surface below when a removable guard rail used on the platform was not replaced after the egg tote was delivered.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On or about 11 August 2015, employees were using the Line 1 conveyor belt without the employer ensuring that the belt was guarded where it runs over the roller, exposing employees to the hazard of being caught in an ingoing nip point.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    On or about 11 August 2015, four (4) oxygen cylinders and one (1) acetylene cylinder were stored together in the maintenance area, without a 20 foot separation or fire resistant barrier, exposing employees working nearby to a fire or explosion hazard.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate.   On or about 11 August 2015, the mezzanine floor in the maintenance area did not contain a load rate limiting informational sticker or placard; exposing employees working beneath to hazards of being struck by falling material or debris due to overloading.
