29 CFR 1926.451(d)(12)(i): When wire rope clips were used on a suspension scaffold, a minimum of three wire ropes clips were not installed. Location: 65 Commonwealth Avenue, Boston, MA. Employees were exposed to fall hazards greater than 10 feet, while working from a suspended scaffold, where only two wire rope clips were securing the tie back cable to the outrigger beams.

Employees were exposed to fall hazards at greater than 10 feet, while working from a suspended scaffold, where vertical lifelines were tied around brick chimneys, light weight guardrail, stair support, and suspension scaffold tie backs.
