29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (a)Innovative Textiles, Inc. at 559 Sandhill Lane, Grand Junction, CO 81501: On and before August 11th, 2015, the exposing employer failed to conduct a yearly review of the energy control procedures. Specifically, the employer failed to ensure that the periodic inspection was performed by an authorized employee other than the ones(s) utilizing the energy control procedure being inspected and that the periodic inspection corrected any deviations or inadequacies identified when new equipment or procedures where identified. The condition exposed employees to potential point of operation hazards when utilizing machinery under the energy control program.

Specifically, the employer did not ensure exposed wiring from the control box on the rewind machine located in the coating/rewind production area was guarded against accidental contact. The condition exposed employees to potential electric shock hazards which working with the rewind machine.

The condition exposed employees working in the area to electrical and potential fire hazards due to improperly installed live electrical cables.
