Manure Management, LLC dba Five Cs Drying located at 9960 Rosedale Rd. Hicksville, OH; On or about August 12, 2015, the employer did not assure a lock and tag were placed on the equipment while performing maintenance. Employees were required to change out the motor on the feed conveyor and did not apply a lock or tag, exposing them to caught by hazards from a rotating shaft.

Employees are required to work around the tractor and mixer wagon and the partially unguarded PTO shaft, exposing them to caught by hazards.
