29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about August 12, 2015, at a worksite located at 29215 Wolf Road, Bay Village, OH 44140, during construction activities including but not limited to roofing activities on a steep sloped roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn during nailing, cutting and weatherproof material installation activities.
