Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) On or about July 22, 2015 for the employee exposed to falls approximately ten feet while driving a JLG Model 1230ES scissors lift in an elevated position to remove tank mold attachments in the manufacturing plant at 113 First Street Neche, ND. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the manufacturers recommendations which states that all persons in the platform wear a full body harness with a lanyard attached to an authorized lanyard anchorage point while operating this machine.

(a) On or about July 22, 2015 for the employees exposed to falls of nine feet while removing or emplacing tank molds on Trolley's 1, 2, and 3 supporting Oven #3 in the manufacturing plant at 113 First Street Neche, ND. (b) On or about July 22, 2015 for the employees exposed to falls over six feet while removing or emplacing tank molds on Trolley's 1, 2, and 3 supporting Oven #2 in the manufacturing plant at 113 First Street Neche, ND.
