29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    This violation occurred on or about August 12, 2015, at a workplace located at 1541 Gram Well, Rockwall, TX; where at least one (1) cornice employee, who was using nail gun, was not protected from eye injuries by wearing safety glasses.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    This violation occurred on or about August 12, 2015, at a workplace located at 1541 Gram Well, Rockwall, TX; where     a) At least three (3) cornice employees, who were working and walking under construction activities, were not protected from head injuries by wearing hard hats.    b) At least five (5) framing employees, who were working and walking under construction activities, were not protected from head injuries by wearing hard hats.    The Gerardo Renteria Dba GRM Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 922054, citation number 1, item number 1 and was affirmed as a final order on 12/10/2013, with respect to a workplace located at 5016 Province Place, Irving, TX; and which was contained in OSHA inspection number 314184599, citation number 1, item number 1 and was affirmed as a final order on 05/10/2012, with respect to a workplace located at 1226 Wedgewood Dr., Forney, TX.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     This violation occurred on or about August 12, 2015, at a workplace located at 1541 Gram Well, Rockwall, TX; where     a) At least one (1) cornice employee, who was working from rafters, performing cornice installation in the front building, was not protected from falling to the ground below.    b) At least five (5) framing employees, who were working from rafters, performing framing installation in the back building, were not protected from falling to the ground below.    The Gerardo Renteria DBA GRM Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 922054, citation number 1, item number 2 and was affirmed as a final order on 12/10/2013, with respect to a workplace located at 5016 Province Place, Irving, TX; and which was contained in OSHA inspection number 314181561, citation number 1, item number 3 and was affirmed as a final order on 05/30/2012, with respect to a workplace located at 700 Osage Drive, McKinney, TX.
