29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about 08/13/2015, at the job site - employees conducting roofing work from a 6:12 sloped roof were exposed to about an 11 feet 8 inch fall hazard, in that, personal fall protection was not used.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: a. On or about 08/13/2015, at the job site - the ladder used to access the roof had structural damage because it was missing the first rung from the bottom.
