29 CFR 1910.29(a)(2)(v): Manually propelled mobile ladder stand was not provided with exposed surfaces free from sharp edges, burrs, or other safety hazards: a) On or about August 14, 2015, located in the maintenance shop and during roof repair activities to semi trucks, the employer did not ensure that the red manual propelled mobile ladder stand used for access was free of bent vertical and horizontal structure tubing that compromised the stability of the ladder and expose employees up to approximately 13 foot fall hazards.

Ladder stands shall have at least two (2) of the four (4) casters and shall be of the swivel type. a) On or about August 14, 2015, during roof repair activities to semi trucks, located in the maintenance shop, the employer did not ensure that all the tubular welded frame scaffolding had at least 2 of the 4 swivel lock casters to prevent scaffolding from unintended movement and potential fall conditions while employees were working a height approximately 13 feet to the ground.
