29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section did not utilize lockout, and the employer did not demonstrate that the utilization of a tagout system provided full employee protection as set forth in paragraph (c)(3) of this section. On or about July 22, 2015, in Department 10 Conomatics, Department 5 Nipple, Department 11 Goss, Department 3 Threaders, and Department 18 CNC, the employer allowed employees covered under the Lockout-Tagout standard to utilize tagout procedures for the control of hazardous energy on a wide assortment of BTB, Nipple Shop Hand and Auto Stamper,Wheelabrator, electrical circuit breakers and Warehouse Stockman/Komatsu machines when the machines were capable of being locked out.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures. On or about July 22, 2015, in the Maintenance Machine shop; the employer allowed employees to be exposed to live parts (three missing breakers) in the 220 Single Phase General Electrical panel box that controls the lights, thus exposing employees to serious electrical injuries.
