29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: (a) Workshop area: On or about August 13, 2015 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to operate the left side of bench grinder without a work rest to support offhand grinding work. (b) Workshop area: On or about August 13, 2015 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to operate the right side of bench grinder without a work rest to support offhand grinding work. (c) Workshop area: On or about August 13, 2015 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to operate heavy duty grinder with a work rest not adjusted to within one eighth of an inch to the wheel.
