29 CFR 1926.960(c)(1)(iii)(A): The employer did not ensure that no employee approached or took any conductive object closer to exposed energized parts than the employer's established minimum approach distance, unless: The employee is insulated from the energized part (rubber insulating gloves or rubber insulating gloves and sleeves worn in accordance with paragraph (c)(2) of this section constitutes insulation of the employee from the energized part upon which the employee is working provided that the employee has control of the part in a manner sufficient to prevent exposure to uninsulated portions of the employee's body). (a) Power Pole Replacement - Job Site 8227 W. Lancaster: On or about August 12, 2015 the employer did not ensure employee(s) replacing old power pole (Pole No. 88T9914) were protected from exposed energized parts on which the employee is working. The employee(s) inadvertently contacted the unguarded/uninsulated 4800 Volt AC primary wire with unprotected/uninsulated parts of the body while setting the wire onto the new pole exposing the employee(s) to an electrical shock hazard.
