29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: The employer is failing to protect employees from explosion and asphyxiation hazards by not providing proper legible signage to warn them of the potential hazards in confined spaces. Permit required confined space danger signs were not posted in appropriate locations including, but not limited to the boot pits and mixing hopper areas. Some of the posted signage was damaged and worn to the point it is illegible.

29 CFR 1910.252(c)(4)(i): Air replacement. All welding and cutting operations carried on in confined spaces shall be adequately ventilated to prevent the accumulation of toxic materials or possible oxygen deficiency. This applies not only to the welder but also to helpers and other personnel in the immediate vicinity. All air replacing that withdrawn shall be clean and respirable.
