29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a) On August 14, 2015, in the attic area of Building 1102, employees worked from a wooden platform that was approximately 12 feet from the floor without guardrails, personal fall arrest system, or safety net system, exposing employees to a fall hazard.

29 CFR 1926.352(a): Readily moveable objects to be welded on, cut or heated were not moved to a designated safe location away from fire hazards:   a)   On August 14, 2015, in the attic of Building 1102, a portable side grinder was used for cutting galvanized sheets of steel in an area that contained combustibles such as cardboard boxes and wood materials.
