29 CFR 1926.950(c)(2)(ii): Before work begins, the contract employer shall advise the host employer of any unique hazardous conditions presented by the contract employer''s work. On or about July 30, 2015, before conducting a power factor test during the installation of a transformer the employer, working as the contractor employer, failed to ensure the host employers employees were advised of the unique hazardous conditions of performing a 10kV power factor test of the transformer.

29 CFR 1926.950(c)(3): Joint host- and contract-employer responsibilities. The contract employer and the host employer shall coordinate their work rules and procedures so that each employee of the contract employer and the host employer is protected as required by this subpart. On or about July 30, 2015, during the installation of a substation transformer the employer, operating as the contract employer, failed to coordinate their work rules and procedures with the host employer for protecting test areas where host employer and contract employer employees were working jointly.

29 CFR 1926.963(b)(1): Safe work practices. The employer shall establish and enforce work practices for the protection of each worker from the hazards of high-voltage or high-power testing at all test areas, temporary and permanent. Such work practices shall include, as a minimum, test area safeguarding, grounding, the safe use of measuring and control circuits, and a means providing for periodic safety checks of field test areas. On or about July 30, 2015, during a transformer installation project, the employer failed to enforce test area safe work practices for the protection of workers from the hazards of 10kV power factor test conducted on the substation transformer.
