Graham Packaging Company, LP, 143 South Lake Road, North East, PA 16428, on or about August 14, 2015 and times prior thereto: The rotating idler end shaft for the in-feed laner / palletizer conveyer line was not guarded. B.) Graham Packaging Company, LP, 143 South Lake Road, North East, PA 16428, on or about August 14, 2015 and times prior thereto: The moving conveyer belt for the in-feed laner / palletizer conveyer line was not guarded on the return side of the conveyor in that a portion of the moving conveyor belt sagged below and outside the frame of the machine.
