29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about August 14, 2015, and at times prior thereto, employees were using an 8 inch ventilation fan where the electric circuit's ground path was not continuous and permanent. This condition exposed employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about August 14, 2015, and at times prior thereto, employees were using a flexible cord with its strain relief broken. This condition exposed employees to electrical shock hazards.
