29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  Warehouse area - On or about August 5, 2015, exit route on the northeast side of the facility was obstructed by permanent stored cabinets, shelves, and pallets.

29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."  Warehouse - The south exit door was not marked by a sign reading Exit. Thereby the employees did not have a visible marked exit door to egress during and emergency.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  Warehouse - On or about August 5, 2015, the employer did not ensure that every employees allowed to operate the Crown and Tusk forklift trucks were trained and certified in accordance with paragraph (l).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  Warehouse Northeast - On or about August 5, 2015, the horozontal metal cutting band saw did not have the unused portion of the blade completely guarded, thereby exposing the employees to the hazards of rotating saw blade.
