29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: Construction site: On or about August 6, 2015 and at times prior thereto, employees were using a pallet on an all-terrain forklift to work at an elevated level; the platform was not approved or allowed by the manufacture. This condition exposed employees to a fall of approximately 4 to 6-feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems: Construction site: on or about August 6, 2015 and at times prior thereto, two employees were working on the roof without any type of fall protection. This condition exposed the employees to a fall greater than six feet. Renew Roofing was previously cited for a violation of this occupational safety and health standard 29 CFR 1026.501(b)(10), which was contained in OSHA inspection number 1055796, citation number 01, item number 001 and was affirmed as a final order on May 26, 2015, with respect to a workplace located at 7200 N Freeway, Fort Worth, TX 76137.
