29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a)  On or about 08-17-15, employees were working on a roof approximately 16 ft. above surface level using incorrectly installed personal fall arrest systems which did not meet the anchorage strength requirements.  Two employees using vertical lifelines were connected to one single D-ring anchor bracket.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  On or about 08-17-15, employees were not using personal fall arrest systems correctly which would have created a greater than six (6) ft. free fall.  Two employees were not adjusting rope grabs on vertical lifelines to decrease approximately 6 to 10 ft. of slack while working on a roof at approximately 16 ft. above surface level.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a)  On or about 08-17-15, employees were utilizing personal fall arrest systems on a roof at approximately 16 ft. above surface level which were last inspected in July 2015.  One Lanyard did not have any manufacturer label and one lanyard had a label that was not legible.
