29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: On or about, 08/03/2015, on the seventh floor roof top, a concrete platform where an air conditioning unit was located, was not guarded. A maintenance employee fell more than 7(seven) feet off the platform to the roof top while in the process of power-washing the unit.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: On or about 08/03/2015, on the seventh floor elevator control room, three elevator control cabinets with exposed electrical wires, tubes, and fuses were not covered. On the date above, three maintenance employees were exposed to electrocution hazards when walking through the control room near the energized cabinets, on their way to the roof top.
