On or about August 17, 2015, at the construction site where an employee was exposed to electrical hazards while using an electrical extension cord where the ground wire had detached from the plug.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a. On or about August 17, 2015, at the construction site where an employee was exposed to electrical hazards while using an extension cord that had the outer insulation pulled back from the female end of the extension cord allowing strain to be applied to the terminal ends in the plug. b. On or about August 17, 2015, at the construction site where an employee was exposed to electrical hazards while operating a Sunbeam brand microwave that had the outer insulation pulled back from the point where the cord comes out from the microwave allowing strain to be applied to the terminal ends in the plug.
