Condition exposed employees to the potential for eye injuries from projectiles to the eyes.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Rascon Bencomo, Ever at 2248 Spruce Creek Drive, Fort Collins, CO 80528: On or before August 15, 2015, the employer allowed the use of a red and a green extension cord with missing ground plugs. In addition, an orange cord connected directly to the Skilsaw, model SHD77M, serial number XXX 791683, that was missing its ground plug. Also, a black cord connected directly to the Dewalt air compressor Model D155146, sn 0037694, and was also missing its ground plug. Both the Skilsaw and the Dewalt air compressor were not double insulated. Condition exposed employees to 120 volt electrical shock hazards.
