29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a. 6781 Hadley Rd., South Plainfield, NJ Employees engaged in construction activities of a commercial building accessed the scissor lift (scaffold) from a distance greater than 2 feet above the work surface by using the guardrails as a means of access. An employee was observed accessing the scissor lift work platform by climbing over the parapet wall and using the lifts' guardrails as a means of access. Violation observed on or about August 17, 2015

An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. 6781 Hadley Rd., South Plainfield, NJ Employees engaged in construction activities of a commercial building were not protected from falls while leaning over the edge of a parapet wall. Employees were observed installing metal studs to a section of the roof while leaning over the edge. Violation observed on or about August 17, 2015
