Queens St. Lindenhurst NY - Employees were walking and working on flat commercial roof approximately 15ft above the ground. The structural stability of the roof and the decking materials had been compromised due to water damage; on or about 8/19/15. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

A demolition plan was not created to determine the structural stability of the roof; on or about 8/19/15. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
