29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard on a construction site in Tuscola, IL and adequate fall protection was not being used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The employer had employees exposed to a fall hazard, while accessing and exiting the roof via an extension ladder that only extended approximately 1.5ft above the upper landing surface of the roof, on a construction site in Tuscola, IL. Roofs by Rodger, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 466040, citation number 2, item number 2A and was affirmed as a final order on August 14, 2012, with respect to a workplace located at 912 N. Chanute, Rantoul, IL 61866. Roofs by Rodger, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 329160, citation number 1, item number 2A and was affirmed as a final order on May 29, 2015, with respect to a workplace located at North of Homer, IL on Rt. 49, Homer, IL 61849.
