29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: Hotel renovation site: On or about February 6, 2015 and at prior times thereto, all electrical outlet boxes in the hallway from the 5th through 12th floors were energized and missing the faceplate covers. This condition exposed the employees to electrical burn and/or shock hazards.
