29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3) of this section) on all open sides:  A.  On or about August 20, 2015, Insulation Warehouse:  The wooden guardrail provided on the storage mezzanine was insufficient in that:  1.  The smooth-surfaced top rail was positioned more than 42 inches nominal from the platform surface.  2.  The post in the center of the long side of the guardrail was cracked.  The rail did not have the strength to withstand the minimum requirement of 200 pounds top rail pressure.  3.  When the mezzanine was accessed by powered industrial vehicles, the front portion of the guardrail was removed, leaving an unprotected edge of more than 9 feet, exposing employees to 8 1/2 foot falls.   B.  On or about August 20, 2015, Garage Door and Gutter Warehouse:    1.  The wooden guardrail provided on the storage mezzanine was not provided with an intermediate rail.   2.  The front portion of the storage mezzanine, near the ladder access, was not provided with a standard guardrail.

29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with the face of the riser at the forward edge of the tread:  A.  On or about August 20, 2015, Insulation Warehouse:  The stair rails to the mezzanine were not provided with intermediate rails, exposing employees to fall hazards.

On or about August 20, 2015, in the following locations, exit doors were not able to be easily opened:  1.  Insulation Warehouse:  The identified emergency exit at the front side of the warehouse was locked with a barrel-lock during building occupancy.  2.  Fireplace Department:  The identified emergency exit door was stuck shut and unable to be easily opened.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  A.  On or about August 20, 2015, in the following locations, exit routes were obstructed:  1.  Insulation Warehouse:  Front, side exit door was blocked by the placement of a Ridgid cutoff saw on a rolling cart.  2.  Garage Door and Gutter Warehouse:  The exit route to the right rear of the warehouse was obstructed by threshold strips that had fallen from a broken box.  3.  Garage Door and Gutter Warehouse:  Exit route to the right rear of the warehouse was blocked by the storage of weatherstripping and a 2x4 board across the aisle.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  A.  On or about August 20, 2015, Garage Door and Gutter Warehouse Shipping Bay:  The Dayton dock light was plugged in and the socket was energized.  The dock light did not have a light bulb in the socket, exposing employees to the energized socket.

29 CFR 1910.304(g)(6)(vi)(C)(8): Portable hand lamps with exposed noncurrent-carrying metal parts that could become energized were not grounded:  A.  On or about August 20, 2015, Garage Door and Gutter Warehouse, Shipping Dock:  A two-wire portable clip on hand lamp attached to the Dayton dock light, was ungrounded.  When energized, energy was transmitted to the metal shade, exposing employees to electric shock hazards.
