(b) Facility: West Wing Receptacle Near Room 116: On or before August 19, 2015, a receptacle subjected to collision damage was not firmly secured to the wall.  (c) Facility: North Wing Receptacle Near Room 226: On or before August 19, 2015, a receptacle subjected to collision damage was not firmly secured to the wall.  (d) Facility: Basement Near the HR Office: On or before August 19, 2015, a receptacle subjected to collision damage was not firmly secured to the wall.
