(a) Uphill Plant, Band Saw with Broken Disconnect Door - On or about August 19, 2015, the stop button at the operator position was not working. The operator had to reach inside the disconnect box on the opposite side of the band saw to shut the saw off thereby being exposed to live electrical parts.
