29 CFR 1926.502(d)(6)(iv): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.   a)      On July 30, 2015 and at times prior thereto; rear roof area; an employee had connected to the horizontal lifeline by using a 10 foot rigging strap.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:   a)      On July 30, 2015 and at times prior thereto; rear roof area; the employer had not installed a horizontal lifeline under the supervisor on a qualified person exposing the employees to a fall hazard while using the personal fall arrest system.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee     a)      On July 30, 2015 and at times prior thereto; rear roof area; employer had not ensured that the lifeline used in the process was capable of supporting 5000 pounds per employee attached.
