29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a) Employees were exposed to head injury while working in a trench and around excavating equipment without protective helmets.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):       a) Employees were not protected from the hazard of cave-ins by shoring, sloping, or other protective means while working in a trench having depths ranging between approximately 7 and 8 feet.      City Construction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 315148403, citation 2, item 1, and was affirmed as a final order on February 7, 2011, with respect to a worksite located at Palmer and Sawyer, Chicago, Illinois.     City Construction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 315154799, citation 1, item 1, and was affirmed as a final order on June 10, 2011, with respect to a worksite located at 2100 S. Leavitt, Chicago, Illinois.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
