29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) West Walnut Street southwest of intersection of Mercer Avenue and West Walnut Street, Kingston, PA - Intersection of Mercer Avenue and West Walnut Street, Kingston, PA - Employees that uncovered a water main while working within an unprotected trench that was six-foot, one-inch (6'1") deep with vertical side walls, exposing employee to struck-by or caught-in hazards, on or about August 19, 2015. b) Intersection of Mercer Avenue and West Walnut Street, Kingston, PA - Employees that uncovered a water main while working within an unprotected trench that was six-foot, six-inches (6'6") deep, eleven-foot (11') long, eight-foot, eight-inches (8'8") wide, and with vertical side walls, exposing employees to struck-by or caught-in hazards, as observed on or about August 19, 2015.
