29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift: Residential building under construction; South Side, Genie Aerial Lift (Model: S-45 S/N:S4513-18716), On or about August 19, 2015, an employee was exposed to a fall approximately 12 feet in height to the unpaved ground level below, while installing a metal tracker frame on a commercial building from an aerial lift without a personal fall arrests system.
