Donnelley & Sons Company located at 1145 Conwell Ave. Willard, OH 44888; On August 19, 2015, the employer did not ensure a employees were protected while working on the Printing Press #333 Folder. Employees were required to make adjustments to the needle inside the folder and did not deenergize the Press, exposing them to in going nip points between the rollers.

An employee sustained injuries to the left forearm when he tripped and lost his balance, sending his left arm into the rollers of the Press #333 Folder.
