29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 7799 Route 57 Front of Building, on or about 8/20/2015: Employees who were installing rubber gaskets near the edge of a commercial building more than 6 feet high without having conventional fall protection, were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 7799 Route 57 Front of Building, on or about 8/20/2015: Employees were accessing the upper work platform by using a ladder that was not extended 3 feet from the landing surface, exposing employees to a fall hazard.
