29 CFR 1910.1030(c)(1)(i):  The employer had not established a written Exposure Control Plan designed to eliminate or minimize employee exposure to bloodborne pathogens:    (a)  Goodwill Industries of Denver at 18355 East 35th Place, Aurora, CO 80011:  On or before July 30, 2015, the employer had not established a written Exposure Control Plan designed to eliminate or minimize employee exposure to bloodborne pathogens.  Employees, as part of their job duties, cleaned spills of blood in restrooms.  The employees were potentially exposed to blood or Other Potentially Infectious Materials (OPIM).  This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(g)(2)(i):  The employer did not ensure that each employee with occupational exposure participated in a training program in accordance with the requirements of the section:    (a)  Goodwill Industries of Denver at 18355 East 35th Place, Aurora, CO 80011:  On or before July 30, 2015, employer did not ensure that each employee with occupational exposure to blood borne pathogens participated in a training program in accordance with all requirements of the section.  Employees, as part of their job duties, cleaned spills of blood in restrooms.  This condition exposed employees to a bloodborne pathogen hazard.    Abatement Note:  Training shall be provided as follows:      1) At the time of initial assignment to tasks where occupational exposure may take place;  2) Within 90 days after the effective date of the standard; and  3) At least annually thereafter.  4) The training program shall contain at a minimum the following elements:  (a)  An accessible copy of the regulatory text of this standard and an explanation of its contents;  (b)  A general explanation of the epidemiology and symptoms of bloodborne diseases;  (c)   An explanation of the modes of transmission of bloodborne pathogens;  (d)  An explanation of the employer's exposure control plan and the means by which the employee can obtain a copy of the written plan;  (e)   An explanation of the appropriate methods for recognizing tasks and other activities that may involve exposure to blood and other potentially infectious materials;  (f)   An explanation of the use and limitations of methods that will prevent or reduce exposure including appropriate engineering controls, work practices, and personal  (g)  protective equipment;  (h)  Information on the types, proper use, location, removal, handling, decontamination and disposal of personal protective equipment;  (i)    An explanation of the basis for selection of personal protective equipment;  (j)    Information on the hepatitis B vaccine, including information on its efficacy, safety, method of administration, the benefits of being vaccinated, and that the vaccine and vaccination will be offered free of charge;  (k)  Information on the appropriate actions to take and persons to contact in an emergency involving blood or other potentially infectious materials;  (l)    An explanation of the procedure to follow if an exposure incident occurs, including the method of reporting the incident and the medical follow-up that will be made available; Information on the post-exposure evaluation and follow-up that the employer is required to provide for the employee following an exposure incident;  (m)An explanation of the signs and labels and/or color coding required by paragraph (g)(1); and  (n)  An opportunity for interactive questions and answers with the person conducting the training session. 29 CFR 1910.132(d)(1):  The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a)  Goodwill Industries of Denver at 18355 East 35th Place, Aurora, CO 80011:  On or before July 30, 2015, the employer did not ensure that the workplace was assessed to determine if hazards were present which would necessitate the use of personal protective equipment.  Employees sort through donated goods that have the potential to contain broken glass, sharp edges, and other damaged items.  This condition potentially exposed employees to cut and/or abrasion hazards.      Abatement Note:  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following:     1)     The workplace evaluated;  2)     The person certifying that the evaluation had been performed;  3)     The date(s) of the hazard assessment; and,  4)     The document identified as a certification of the hazard assessment.
